Quick answer

Most back pain and sciatica comes from an irritated disc or nerve in the lower spine — and the great majority settles with the right medication, physiotherapy and, in some cases, an injection. Surgery is needed only in a small number of cases, and Dr. Vishal tells you honestly whether yours is one of them.

The basics

What causes back pain & sciatica?

Your lower spine is a stack of bones (vertebrae) cushioned by soft discs. When a disc bulges or wears, it can press on a nerve — causing back pain, or pain that shoots down the leg (sciatica). Muscle strain, poor posture and age-related wear are the most common triggers.

The good news: the spine is designed to heal. Most disc and nerve irritation settles on its own with time and the right care — which is why the first step is almost never surgery.

Signs your back needs attention

Most back pain eases in a few weeks. See a specialist sooner if any of these are true — and treat the last two as an emergency.

No pressure

Do you actually need spine surgery?

Without surgery
  • A short course of the right medication
  • Guided physiotherapy & core-strengthening
  • Posture, activity & ergonomic changes
  • A targeted injection for the right cases
  • Time — most disc pain settles in weeks
When surgery is needed
  • A nerve is pinched with real, progressive weakness
  • Sciatica that is severe and not settling with proper care
  • An MRI shows a problem that matches your symptoms
  • Loss of bladder or bowel control (an emergency)
  • Conservative care has genuinely been tried and failed
Dr. Vishal treats the spine conservatively first — surgery is a last resort, not a first offer. He reviews your MRI with you, explains exactly what is (and isn't) causing your pain, and recommends an operation only when it will clearly help.

The approach

How Dr. Vishal treats it

Care starts with a careful examination and, when needed, an MRI to see exactly which disc or nerve is involved — because back pain has many causes and the right treatment depends on the real one. Many patients simply need the correct medication and a structured physiotherapy plan.

When a nerve is genuinely compressed and not recovering, modern minimally invasive techniques can relieve the pressure through a small incision, with less pain and a faster recovery than traditional open surgery. The goal is always the least intervention that gets you back to normal life.

No jargon, no pressure

See exactly which nerve is affected before any surgery

Dr. Vishal walks you through your own X-ray or MRI, shows you exactly what's going on, and lays out every option — surgery and non-surgery — so the decision stays yours.

  • You see your own scan — pointed out in plain language, not medical jargon.
  • Every option on the table — surgery, physiotherapy, medication or simply waiting and watching.
  • You make the final call — no pressure, and never a rushed decision.
What to expect

Getting back to normal

For most back pain and sciatica, recovery is steady and non-surgical — this is the usual path.

1
Week 1–2
Calm the pain

Medication and rest settle the acute pain; gentle movement is encouraged, not complete bed rest.

2
Week 2–6
Move & strengthen

Guided physiotherapy rebuilds core and back strength; most sciatica eases over this period.

3
Week 6–12
Back to routine

Normal work, walking and daily activity return, with posture and lifting technique corrected.

4
Ongoing
Stay strong

Simple exercises and habits keep the back healthy and cut the chance of it coming back.

"I had ongoing pain in my left hip joint, but the X-ray was normal so no one could find the cause. Dr. Vishal explained the difference between a nerve problem and a bone problem, and examined it properly. I'm much better now, and the problem is nearly gone."

"My brother had a leg fracture. Another hospital kept him in ICU for a week and was running up unnecessary bills. Dr. Vishal read the reports, explained there was no abdomen or head injury, operated on the fracture — and 10 days later my brother is comfortable. His advice is genuine."

Frequently asked

Back pain & sciatica — your questions

Is my back pain a slip disc?
Maybe — a "slip disc" (a bulging disc pressing a nerve) is a common cause, especially when pain shoots down the leg. But many back pains are muscular. An examination, and an MRI when needed, tells the difference — and most disc problems still improve without surgery.
Do I need surgery for sciatica?
Usually not. The large majority of sciatica settles with medication, physiotherapy and time. Surgery is considered only for a nerve under real, progressive pressure, or when good conservative care has failed. Dr. Vishal advises honestly either way.
Do I need an MRI?
Not always. For simple, recent back pain an MRI often isn't needed at first. It is advised when pain is severe, not settling, or when there are nerve symptoms like leg weakness — so treatment targets the real cause.
What red flags should I not ignore?
Leg weakness, a dragging foot, numbness around the saddle area, or loss of bladder or bowel control need urgent attention — treat the last of these as an emergency. Back pain with fever, or after a serious fall, should also be checked quickly.
Is it covered by Ayushman Bharat or insurance — and what will it cost?
Ayushman Bharat, cashless insurance and other schemes are accepted where applicable. Most back-pain care is non-surgical and low-cost; if a procedure is needed the team gives a clear written estimate first — message on WhatsApp.
Will exercise make it worse or better?
The right movement usually helps — prolonged bed rest can actually slow recovery. Dr. Vishal and the physiotherapy team guide exactly which exercises to do, and which to avoid, for your specific problem.
