A complex shoulder fracture after an accident is a break of the top of the arm bone (proximal humerus), the shoulder blade (scapula), or the collarbone that is displaced, in several pieces, or dislocated as it broke. The choice is between fixing it with plates and screws and, when the ball of the joint is destroyed (often in older patients), a shoulder replacement. Starting gentle movement early is what keeps the shoulder from freezing stiff.
The shoulder is a ball-and-socket where the top of the arm bone (the proximal humerus) sits against the shoulder blade. In an accident this can break into several fragments, the ball can split, or the shoulder can fracture and dislocate together. The clavicle (collarbone) and scapula can break too, and the nerves and vessels that run past the shoulder can be injured.
What makes it complex is the number of pieces and whether the ball of the joint survives.
